Required Immunizations For All Students
• DTP (Diptheria, Tetanus, Pertussis): 3 dates• Tdap: 1 date within the last 5 years
• Polio: 3 dates
• Measles: 2 dates
• Mumps: 2 dates
• Rubella: 1 date
(2 MMR's meet the Measles, Mumps, Rubella requirement)
• Hepatitis B: 3 dates for all students born after July 1, 1994
Additional Immunizations Required
If you are an international student• TB Skin test (PPD): 1 date within the last year
If you are applying to the nursing program
• Varicella Titer
• Rubella Titer
• Hepatitis B: 3 dates
• TB skin test (PPD): 1 date within the last year
If you are an Athletic Training Student
• Hepatitis B: 3 dates
• TB skin test (PPD): 1 date within the last year
